We understand that it may be frustrating for you to be asked for certain germane information on each visit by our staff, but we need you to know that it is necessary as our EMR system commands certain data be completed.
Your insurance payers put demands on physicians and one of them is to collect your insurance information at every visit including a copy of your card so that we have the most correct, and current, information to file your claim for you.
As your neurologists, we must have you complete a medical form at every visit to ensure that we have the most current medical information in order to provide the highest quality care to you. Therefore, we ask for your understanding and cooperation with information that we must collect at the time of service so that your visit goes smoothly and is done in the most timely fashion.
