Insurance and Payment Information Telephone Calls Prescription Refills Patient Information Form What to Bring to Your Appointment
A nonemergent appointment is usually scheduled within two weeks. If you need to cancel an appointment, a 24-hour notice is appreciated. We ask that you please arrive 15 minutes before your scheduled appointment time so that you can complete the patient information form and any other forms or germane information required for each visit. We make a sincere effort to adhere to our appointment schedule, but emergencies do occur, and delays are sometimes unavoidable. Insurance and Payment Information We participate with Medicare and many of the commercial and workers' compensation managed care health plans. However, we encourage you to check your insurance provider manual to be sure your plan allows you to be seen by one of our providers. Various plans do require a referral, so please be sure to obtain one prior to your visit in order to avoid having to reschedule your appointment. We process all insurance claims if we are provided with the appropriate information. If you have any questions regarding your coverage and/or benefits, please direct them to your employer or insurance company. Most health plans require that you pay a co-payment and/or deductible, which is expected at the time of your visit, and any balance not covered by your insurance company will also be your responsibility. For your convenience, we accept all major credit cards for all payment types. If you have any billing questions or concerns, our billing department will be happy to assist you. We thank you for your understanding and cooperation as we follow the many rules and regulations put upon us by the insurance companies. We have an automated phone system in order to help expedite your call. Please listen to the option menu, select the option that meets your needs, and you will then be connected accordingly. If it is an emergency, please dial "0" and state your emergency to the staff member who will then direct your call appropriately. If you select the option for a return call from your physician, it is imperative that you leave a detailed message so that all necessary records and reports will be available when your call is returned. Unless emergent, all calls are returned throughout the day up to 7:30p.m. Important Information Regarding Prescription Refills All prescription refill requests are processed during normal business hours, Monday thru Thursday from 9 a.m. – 4 p.m., and Friday from 8 a.m. – 3:30 p.m. Please allow 2 business days for your request to be completed due to insurance companies mandating prior authorizations on many medications.
